1. On page 63922, in the first column, the docket number is corrected to read as set forth above.

2. On page 64022, in the second column, in Sec. 417.233(c)(5)(ii), in the fourth line, after ``the'' add ``EFG''.

   Appendix A to 417 [Corrected]
