A person required to file a statement pursuant to section 38a-130 of the Connecticut General Statutes shall furnish the required information on Form A (Appendix A of sections 38aForms - general requirements38a-138-1 to 38aSeverability38a-138-16, inclusive, of the Regulations of Connecticut State Agencies). Such person shall also furnish the required information on Form E (Appendix E of sections 38aForms - general requirements38a-138-1 to 38aSeverability38a-138-16, inclusive, of the Regulations of Connecticut State Agencies). The applicant shall promptly advise the Commissioner of any changes in the information so furnished on Form A arising subsequent to the date upon which such information was filed with the Commissioner. The Commissioner shall hold a public hearing within thirty (30) days from the date at which the Commissioner determines that the Form A is complete in all respects.
